The name and contact information of the person who provided the procedure. 4. Documentation that an explanation of the procedure and information about any risks associated with the procedure were communicated to client. 5. Dates of the procedure. 6. The lot numbers and expiry date of the pre-packaged sterile equipment used in the procedure.
