- Operator had not submitted the Personal Service Notification to notify the AHS about change of ownership/opening of new business.PSN was submitted by PHI on behalf of operator.

- Written procedures related to the services provided at the facility were not established by the operator:Based on the services offered at the facility following procedures are required.1) Written procedures for skin cleansing.2) Written procedures for accidental skin cut and punctures.Templates will be provided for reference.

- Operator was not fully aware of Personal Service Standards and Regulations, classification of equipment and requirements for disinfectants.Education was provided to operator.Resources will be shared for reference.

- Some visibly dirty equipment was observed to be stored along with clean equipment in waxing area of the facility.Make sure to store the dirty and clean equipment separately.Discussed the labelling of containers for easy identification of clean and dirty equipment.

- 1) HLD was being re-used for disinfection of semi-critical equipment, without testing it.2) Logs for HLD testing were not available.Make sure to test the HLD with required test strips on routine basis and maintain the records.HLD log sheet will be provided with this e-mail.**Please follow Manufacturer instructions for reuse period, contact time and pre-cleaning procedures**

- The tools reprocessing area was present near the client's handwashing station.Reprocessing areas must also be protected from client access. The goal is to limit potential client contact with reprocessing chemicals and equipment to prevent client injury/infection and to prevent clients from contaminating equipment. Client restriction could be accomplished through many different measures, such as posting a sign indicating the sink is for employee use only.

- Unlabeled cosmetic bottles were observed in the Pedicure carts.Make sure to label all the cosmetic and chemical bottles to identify their contents and avoid cross -contamination.
