However, the Vietnamese AHS written procedure template was blank (not completed).** Reviewed the required procedures with Thu, Please complete the required pages and post to the staff room (Skin cleansing, and sink decontamination procedures). Send a photo of the completed corrected actions.Ensure the sink is cleaned and disinfected before and after it is used for equipment reprocessing.

- Single use wooden wax stick was reused for waxing.PHI found in the garbage, one wax stick with at least three large fabric wax strips, suggesting double dipping of the wax stick Personal service worker stated that it was from another worker waxing her legs. The worker stated that they don't double dip for clients.PHI explained that there must not be any double dipping, regardless of who the service was for, to reduce the risk of cross-contamination.- Single use mascara wands that were covered with hair and debris were found in the waxing room.Corrective Action:- Discard contaminated wax, and refrain from double dipping.- Discard used, single use, mascara wands.

- Visibly dirty equimpent (tweezers, scissors, brushes) were found in the waxing room. These items were stored in a visibly dirty and sticky glass jar.Corrective Actions:- Clean and disinfectant all reusable equipment in the waxing room, including containers.Recommend placing two containers in the waxing room; one labeled as "clean", the other labeled as "dirty" or "used".Clean equipment should be placed in the "clean" container. Once used, equipment must be placed in the "dirty" container. This will help prevent cross contamination of dirty and clean equimpent, as well as encourage workers to bring dirty equipment to the reprocessing area to be cleaned and disinfected.

- Preempt CS20 high level disinfectant was expired (April 30, 2025).Corrective Action Taken on June 19, 2025:- New Preempt CS20 purchased (exp Febr 6, 2026)Daily disinfectant test results were incomplete and the practice of documented results had ceased.Corrective Actions:- Please acquire a new bottle of disinfectant.- Test the disinfectant daily and document the results in the high level disinfection log. Use a new log sheet every 14 days.- Ensure the solution is not used more than 14 days as per the manufactuer's instructions.

- - Squeeze bottles located around the pedicure and manicure stations containing cosmetics were not labeled, or the labeled was faded.- Berrywell Augenblick Hair dye was being used for dying the eyebrows and eyelashes, however the labeled cleared stated that the "product must not be used for dyeing the eyebrows or eyelashes, to do so may cause blindless."Corrective Actions:- Ensure all contains of cosmetics are labeled with their product identifier, and the date they were filled.- Remove the Berrywell Augenblick hair dye from the facility.
