      • Before providing a personal service, the operator of a personal service setting or the person who will be providing the personal service shall obtain the name and contact information of the person seeking the service.

    • Did operator maintain setting in a safe and sanitary manner?
      • Floors, walls, fixtures, and furniture must be in good repair, clean and made of smooth and impermeable material
    • Did operator provide all required supplies at the designated hand washing sink?
      • Ensure there is soap in a dispenser in close proximity to handwashing sink.
    • Did operator discard sharps immediately after use into a sharps container?
      • Ensure that, immediately after use, all sharps (e.g., razor blades, credo blades, needles, lancets, scalpels) shall be disposed of in a sharps container that is located in close proximity to where the personal service is provided.
      • Provide a puncture and leak resistant container with a fill line designed for the safe disposal of sharps.
      • Ensure that sharps containers are compliant with standards of the Canadian Standards Association.
      • Ensure that sharps containers are disposed of in accordance with the requirements of the Environmental Protection Act, its regulations, and any other applicable law of Ontario.
