    • Did operator provide all required supplies at the designated hand washing sink?
    • Did operator keep records related to accidental exposures to blood or body fluids?
      • Ensure the records include: date of the accidental exposure, service being provided, part of the body that was exposed, name and contact information of person who provided the service, name and contact information of the person exposed, action taken by the service provider.
      • Ensure the records related to accidental exposures to blood or body fluids are kept for a total of three years in a secure location. One year stored at the personal service setting or until the setting ceases to operate. Retained and kept readily available for at least an additional two years.

    • Did operator post results of inspection in accordance with inspector's request?
    • Did operator clean reusable equipment?
      • Ensure equipment is soaked in detergent and water, an enzymatic cleaner, or soaking solution if equipment cannot be cleaned directly after use
      • Ensure all reusable equipment is cleaned between each use.
      • Disassemble reusable equipment prior to cleaning.
      • Provide and use cleaning supplies (e.g. scrub brush, detergent) and ensure they are easily accessible.
      • Clean reusable equipment in the designated cleaning sink.
    • Did operator follow proper cleaning and disinfection steps?
      • Provide an area where all cleaning and disinfecting of reusable equipment takes place. This area shall be next to the cleaning sink and have adequate counter space.
      • Clean reusable equipment with a scrub brush and detergent.
      • Rinse cleaned equipment.
      • Dry equipment prior to disinfection.
      • Disinfect reusable equipment with appropriate disinfectant required for the equipment.
      • Provide a disinfecting container with a lid or cover that is capable of allowing equipment to be fully immersed in the disinfectant.
      • Dry equipment prior to storage.
      • Store cleaned and disinfected equipment in a sanitary manner.
      • If reusable containers are used, containers are to be emptied, cleaned, disinfected, rinsed, and dried before refilling.
      • Ensure prepared disinfectant solutions are not topped up with fresh solution (i.e., do not add new solution to the current solution)
