- The spoon used to take out the orange scrub is heavily soiled. Ensure reusable tools are regularly cleaned and disinfected to remove debris and contamination. Please clean.

- Several cosmetic products were decanted into another container and did not have a label. Cosmetic products must be properly labeled for staff awareness and safe usage of the product. Please label.

- The outside of the wax warmer is heavily soiled with wax debris. The storage rack is also heavily soiled with wax debris. All work surfaces including equipment must be maintained in a clean and sanitary manner. Please clean.

- 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
- Alcohol is not being used on the skin prior to waxing, or mani/pedi services. Skin must be treated with alcohol or other antiseptic prior to all services.
- 13. Is single-use or uncleanable equipment being used as intended (Critical)?
- Many files and buffers were found onsite that were covered in dust and were clearly being reused. Discard all of these items immediately. Continue to discard products after each client.

- Tools were visibly dirty throughout the facility, both at the manicure and pedicure stations and in the back room in the 'clean storage' unit. Drill heads were not being cleaned after use to remove excess dust, nor were the electric file machines. - Tools must be CLEANED after each use before disinfection with detergent and water. Follow the steps on the Preempt CS20 bottle.
- 20. Is reusable equipment disinfected properly?
- 1) The CS20 disinfectant onsite was located nowhere near the reprocessing area, and was expired since May 2023. 2) Tools at the manicure station were visibly dusty and used, and remained at the station rather than being taken away for cleaning and disinfection. 3) Filing heads for electric file were being soaked in a dirty container of alcohol. These are metal tools that must be reprocessed at the sink, and cannot be left soaking in alcohol. Remove alcohol bowls asap.4) The waxing bed was visibly dirty. Sheets appeared to not have been changed in some time.Remove all the sheets and pillows. Use bench without covers and find a vinyl pillow or a moisture resistant pillow case. There was no disinfectant in the area. Bring Preempt RTU here and disinfect the area after each use. Alcohol is only acceptable for disinfecting skin surfaces. Reprint the procedures for cleaning tools and disinfecting them and post them at the reprocessing sink.

- There was lots of dust buildup at the manicure stations. All areas where services are provided must be cleaned and disinfected with Preempt RTU after each service.
