- A used nail file (single-use item) was noted in nail station 1 (close to the entrance).- Operator discarded it.Ensure that disposable/single use items are discarded immediately after use to prevent potential cross-contamination, do not leave or store them at workstations. Follow manufacturer's instructions for reprocessing items specified as re-usable.

- Semi-critical tools (nail clippers) in the clean reprocessed area were observed to have debris.- Please clean thoroughly and repeat reprocessing/disinfection.Ensure to properly clean and visually inspect equipment before disinfection to prevent cross-contamination.
- 20. Is reusable equipment disinfected properly?
- 1. The steps for reprocessing semi-critical instruments are not being correctly followed. - Wash tools with soap and water, rinse, dry and fully submerge in undiluted high-level disinfectants for the required contact time provided by the manufacturer. Rinse after disinfection and store in a clean/dry location.Ensure that all semi-critical instruments are reprocessed following proper procedures to achieve adequate disinfection and prevent potential risk of infection transmission.2. The high-level disinfectant in a soaking tray with tools did not meet manufacturer's testing requirements as there was a delayed test-strip reaction time of approximately 50 seconds instead of 35 seconds. Operator said they dilute the PREempt CS20 with water.Ensure that high-level disinfectants are maintained according to manufacturer's instructions. Do not dilute with water.- Discard and replace the high-level disinfecting solution.

- Contact time for high-level disinfectants is not in accordance with the manufacturer instructions. Instruments are being immersed for only 15 minutes instead of the required 20 minutes.- Ensure that semi-critical tools are fully immersed in undiluted high-level disinfectants for 20 minutes. This prevents inadequate disinfection, thereby increasing the risk of infection transmission.

- Semi-critical tools observed to be not fully submerged in the disinfectant solution and therefore inadequately disinfected.- Ensure to fully submerge instruments in the disinfectant for the required contact time provided by the manufacturer. Rinse after disinfection.

- 1. A basket containing several rusted/damaged tools was observed to be stored on the lower shelf of a cart used for clean instruments in the reprocessing area. There was no 'do not use' label.- Discard rusted and damaged instruments.Ensure that only instruments in good condition are stored in the reprocessing area and facility for client use. Instruments in poor condition pose potential risk of infection transmission as they cannot be effectively cleaned and disinfected.
