      • 1. Surface and equipment disinfectant, cavi wipes, was expired since May 2018.-Please procure and use effective disinfectant to ensure proper cleaning and disinfection of equipment and facility's surfaces.2. Reusable laser handle covers were being cleaned/disinfected with bleach solution and alcohol.-Please use disinfectant with health Canada's DIN such as preempt RTU to disinfect these covers.

      • There was no sharp container available in the facility, shaving razors were noted onsite.-Please provide suitable puncture resistant containers to ensure the razors are stored and disposed in proper manner.

      • Damaged treatment chair, with torn surfaces, was observed in the facility.-Please repair or replace the damaged chair to ensure smooth, non-porous and easy to clean surface.

There was no Certificate of registration available for the facility's Class IV laser machines.-Please register the indicated laser machines with AASP and ensure the Certificate of registrations are accessible onsite.
