- The procedure provided for reprocessing reusable semi-critical tools was not completed properly. Follow the below provided step-by-step instruction for reprocessing the semi-critical tools. Ensure instructions are translated to language easily understood by staff and post instructions on the wall at the reprocessing sink area. 1) disassembled to its simplest component parts for cleaning2) wash using soap to remove soil, and 3) rinse the equipment thoroughly4) dry using clean towel or paper towel prior to disinfection 5) immerse equipment in the high-level disinfectant, until contact time specified by the high-level disinfectant manufacturer's instructions.6) high-level disinfectants must be rinsed off 7) air-dry equipment on a clean surface

- The wrong test strip was being used for measuring the concentration of the high-level disinfectant used for reprocessing reusable tools. Provide the appropriate test strips for measuring the high-level disinfectant solution.
