      • Disinfectant was being mixed with alcohol. Proper disinfection usage on Semi-Critical, and non-critical instruments is not demonstrated.Staff were not able to demonstrate proper knowledge on manufacturer's instructions of disinfectant.Dec 4: Violation still outstanding.

Single use recirculation jets were observed to have moisture inside. These are single use and cannot be cleaned or re-used. Either discard after each use, or obtained re-usable that can be cleaned and disinfected after each use.

      • Reprocessing was noted to be done in the bathroom. Ensure reprocessing is done in a safe area. Operator said that the toilet is not used. Operator will have the toilet removed. Please let the undersigned know when toilet has been removed.October 06, 2023: Reprocessing of tools was still being done in the out of use toilet. The toilet is yet to be removed. Please note that this is an ongoing violation and must be corrected immediately. Reprocessing of tools cannot be done in a sink located inside a toilet. October 16, 2023: Reprocessing of tools was still being done in the out of use toilet. Note this outstanding violation is not corrected, further enforcement actions may be used.

      • The drain plugs and plastic seals in several foot bowls were unclean. Slimy dirt/debris accumulation and hairs were observed.Remove all drain plugs, have them cleaned and disinfected and ensure the entire bowl is always maintained in a sanitary condition.
