      • Ensure that, immediately after use, all sharps (e.g., razor blades, credo blades, needles, lancets, scalpels) shall be disposed of in a sharps container that is located in close proximity to where the personal service is provided.
    • Did operator follow proper cleaning and disinfection steps?
      • Dry equipment prior to disinfection.
      • Store cleaned and disinfected equipment in a sanitary manner.
      • If reusable containers are used, containers are to be emptied, cleaned, disinfected, rinsed, and dried before refilling.
    • Disinfectants: Did operator follow manufacturer instructions?
      • Follow manufacturer’s instructions for contact times of disinfectant product(s) where available
    • Did operator disinfect equipment and surfaces?
      • Intermediate-level disinfect equipment that are intended to contact only intact skin but may accidentally come in to contact with non-intact skin or mucous membranes or penetrate the skin (e.g., nail clippers, cuticle nippers, reusable dremel bits, ear piercing device, single-use ink caps, tweezers and scissors used in hair removal services).
      • High-level disinfect equipment that contact mucous membranes or non-intact skin during use but do not ordinarily penetrate the skin or enter normally sterile areas, (e.g., grater-style foot files, comedone extractor with loops on both ends, and diamond microdermabrasion tips).
      • Provide a high-level disinfectant and ensure it is easily accessible. High-level disinfectant: Mix 125 ml (½ cup) of 5.25% bleach with 1 L (4 cups) of cold water. Ensure equipment is fully immersed in the disinfectant for a minimum of 10 minutes. Prepare a fresh bleach and water solution daily.
    • Did operator keep records related to accidental exposures to blood or body fluids?
      • Ensure the records include: date of the accidental exposure, service being provided, part of the body that was exposed, name and contact information of person who provided the service, name and contact information of the person exposed, action taken by the service provider.
      • Ensure the records related to accidental exposures to blood or body fluids are kept for a total of three years in a secure location. One year stored at the personal service setting or until the setting ceases to operate. Retained and kept readily available for at least an additional two years.
