      • Observation in 2023:The leather on several pedicure chairs is peeling off.Observation in 2025:The leather on several pedicure chairs is peeling off - Paula indicated that these are no longer used. Please remove them.

    • 17. Is reusable equipment cleaned properly (Critical)?
      • There are several cuticle cutter and nail clippers that are rusted. Please discard do not use rusted tools on clients.
    • 20. Is reusable equipment disinfected properly?
      • 1. The CS20 in the container that's used for disinfecting nail tools had failed the test strip test.2. The bottle of CS20 had expired in April 2024.3. There is no monitoring log for reusing high level disinfectant. 4. Test strip had expired in September 2023. 5. Scissor and tweezer in the waxing room are disinfected with alcohol.Please immediately obtain appropriate high level disinfectant to disinfect the nail instruments, as well as the tweezer and scissors in the waxing room. Obtain new test strip and record test results onto the monitoring log.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Alcohol without a Drug Identification Number is used as a surface disinfectant. Please use only disinfectant that has a DIN. Mr. Clean disinfectant cleaner was available and acceptable to use for surface disinfection.
