- Accidental blood and body fluids exposure/cleanup written procedures were not observed. Please create accidental blood and body fluids exposure/cleanup procedures.
- 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
- Sterility assurance documentations were not being maintained. This was discussed with the operator during the inspection. The operator agreed to maintain sterility assurance records.
- 25. Are the equipment manufacturer's instructions for use of specific equipment available and followed?
- 1. It was noted that the facility was not following Manufacturer's Instructions For Use (MIFU) for both Botox and Dysport. The facility was using saline solution with preservatives to reconstitute these neuromodulators. The facility was also storing reconstituted neuromodulators for 30 days. Please follow manufacturer's instructions for use for these neuromodulators. Please acquire preservative free saline solution for reconstitution purposes. Please use the reconstituted neuromodulators within 24 hours as per MIFU.2. MIFU of one of the neuromodulators were not observed at the time of inspection. Please ensure MIFU is available for all products.
