      • It was noted that facility specific written procedures were not observed.Please create facility specific written procedures with the provided templates.

      • The hand washing sink/reprocessing sink was missing a paper towel dispenser. The washroom's paper towel was not in the dispenser, and there was evidence of hand towels being re-used.

      • Unapproved hand sanitizer was found at both nail stations. October 4th: one nail station has an approved hand sanitizer with an NPN, one does not and must be replaced.

      • Tweezers and scissors throughout the facility were observed visibly dirty, some with hairs stuck to them and waxing residue. It was impossible to determine what items had been "cleaned" and reprocessed, and what had not.October 4: three tweezers in the right hand facial room were found stored visibly dirty. Operator was instructed to clean and reprocess these items.

Please maintain high level disinfection logs using the templates provided.

      • The pedicure baths are not being washed with a detergent that prevents biofilm accumulation. CS20, though it has a TB claim, is for the soaking of instruments and cannot be sprayed into the basin. Parts must be removed, cleaned, and the water must recirculate using a product such a PREempt Concentrate or other.

      • Single use disposable items (popsicle sticks for waxing, q-tips, cotton swabs, boxes of gloves with items falling into the openings, cuticle sticks, blades for calluses, lancets, toothpicks, eyelash under eye pads) were being stored in a way that allowed contamination. These items must be stored in a sanitary way to prevent contamination.

      • Chemicals were observed decanted into various unlabeled bottles. Any product not in its original bottle must be labelled with the contents and if applicable, expiration date.
