- The facility did not have written procedures on skin cleansing and accidental skin cuts or punctures.AHS templates were provided to the operator.

- Hair with debris had tangled onto the pucks in the pedicure basins.The pucks were removed during the inspection.Disassemble equipment to its simplest component for proper cleaning and disinfection.

- The high-level disinfectant log was not completed daily. There were no records for 3 days in August. Monitor the disinfectant daily and ensure written records are maintained. Also, indicate when the disinfectant is replaced.
