    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 1. A portion of the newly opened dermal fillers (i.e. Juvederm, Skinvive, Teoxane) is being stored in separate syringes via a connector for client touch-ups within the subsequent days. Please note that these dermal fillers are intended for single use only and the remaining unused product must be discarded. Please incorporate these corrective actions into the existing policies and procedures regarding the administration of dermal fillers and the reconstitution of neuromodulators, to ensure compliance and client safety.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • There is a large decorative rug in the client washroom. To maintain a hygienic envrionment, please remove rug to allow flooring in the washroom to be easily cleanable.

    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • 1. Staff indicated that they had been using saline with preservatives to reconstitute the neuromodulators. Only Saline 0.9% Bacteriostatic with Preservative was available at time of inspection. Please follow the manufacturer's instruction for use which specify the use of preservative-free saline for reconstituting products such as Botox, Dysport, Nuceiva that are used at this facility.2.       • 1. Only Saline 0.9% Bacteriostatic with Preservative was observed in the storage room. Please follow the manufacturer's instruction for reconstitution. Botox, Dysport, Nuceiva all states to reconstitute with preservative-free saline.2. Pre-loaded syringes are stored in the fridge with date labeled past 24 hours of use. Once reconstituted, store the neuromodulators be in a refrigerator at 2–8°C and used within 24 hours, as per Botox, Dysport and Nuceiva's MIFUs.

      • The Caviwipes in each treatment room were expired.Replace the Caviwipes. Ensure the disinfectants used are always within expiration.
