- Hairbrushes and clippers stored at the hair stations were covered in hair and not clean. Tweezers and other esthetic tools stored in the wax room were visibly dirty.

- Preempt CS20 high level disinfectant was expired. No high-level disinfectant available for disinfecting semi-critical lash tools. All lash tools must be cleaned and disinfected prior to the next lash service. No disinfectant available for disinfecting electric clippers. Electric clippers must be cleaned and disinfected prior to use on the next client. The bottle of Barbacide low level disinfectant was running low.

- Surfaces throughout the facility were not clean. Hair stations were covered in hair. Hair sinks and chairs were visibly dirty. Surfaces in the wax room were visibly dirty.
