- The needle lot numbers and the expiry date for the Botox needles were not being recorded.- Please record the lot numbers and the expiry date for the needles used on each client to ensure tracing back is possible in an event of recall.

- The reprocessing was being done in the staff lunchroom, the sink was being used for non-compatible activities such as washing utensils.- Please move the reprocessing to the facial room sink. Ensure that the sink is decontaminated between handwashing and reprocessing equipment following the written procedures.
