      • Written procedures were not provided for Handwashing and Equipment Reprocessing Sink Decontamination. - Ensure to fill out the written procedures. A template is attached in the email.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • The needle lot numbers and the expiry date for the Botox needles were not being recorded.- Please record the lot numbers and the expiry date for the needles used on each client to ensure tracing back is possible in an event of recall.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • The reprocessing was being done in the staff lunchroom, the sink was being used for non-compatible activities such as washing utensils.- Please move the reprocessing to the facial room sink. Ensure that the sink is decontaminated between handwashing and reprocessing equipment following the written procedures.
