      • Sterility assurance information is not being retained for all clients who receive services that use sterile product- Ensure sterility assurance information is documented for all clients. - Records should include at minimum the lot and expiry dates from sterile products with client contact information

      • 1. Multiple bottles of ink are past their expiry dates and there are inks with no lot or expiry dates indicated on the label or bottle- Ensure inks are discarded and/or removed from main supply (and only used for practice not on people) once they are expired. - Acceptable tattoo inks will have lot and expiry numbers and indicate that they were sterile until the point of use. 2. Decanted product bottles such as for distilled water and green soap mixtures, are not being labeled with contents- Ensure all bottles and containers filled with cosmetic products are properly labeled.
