      • Written procedures for preparation of skin; client care; and post care instructions were not available.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • - Cleansing of the skin is not being done prior to a service that may involve contact with mucous membrane, or that will puncture or may potentially break skin. The operator was advised to acquire a cleanser.-The antiseptic products available in the facility were either expired or did not have a DIN or NPN. The operator was advised to acquire a new antiseptic product with a DIN or NPN.
    • 11. Is post-service care provided by personal service workers following any personal service or accident where a client's skin or mucous membrane is cut or punctured during a personal service, or when energy-emitting equipment is used?
      • -Post-care instructions were not developed for the services where a client's skin or mucous membrane is cut or punctured during a personal.The operator was advised to develop post-care instructions.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • - Client records were not observed. Ensure client records are being maintained for a period of at least two years.- Client records that include adequate information were not available. Client records must include full client name, contact information of client, date(s) of personal service, description of personal service, and name of personal services worker who provided the personal service. Ensure appropriate consent forms are developed and adequate records for each client are maintained.-Needle cartridges used in the facility do not have enough information to keep sterility assurance documentation. The operator was advised to provide more information on the pre-sterile needle cartridges being used in the facility or acquire new pre-sterile needle cartridges that contains manufacturer's information, lot number and expiry date.
