- There was evidence that additional personal services were provided beyond hair. Please submit an updated personal services notification form.
- 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
- Hand soap was not provided at the hand washing station. Please provide hand soap in a suitable dispenser.
- 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
- There was an accumulation of hair inside storage drawers. Please clean.
- 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
- 1) Clippers and other implements were stored on yarn doilies that were contaminated with hair. Surfaces must be easily cleanable.2) A large surface area of the service chair was repaired with tape. Please properly resurface or replace the chair.
