- High level disinfectant (Pre empt CS20) is not tested with the appropriate test papers (AHP 7%). **HLD logs were incomplete, with no pass/fail test. Proper method to log the HLD sheets were discussed with operator. Operator was instructed to purchase the test paper and follow the manufacturer instruction for disinfectant's use.

- Some of the reusable tools at the work-station drawers were stored inside the containers with nail dust in them. Instructions provided. Ensure there is proper separation of cleaned and dirty tools. Take all the used reusable tools to the reprocessing area after each client service. Properly store the processed tools inside the clean containers/drawers (ensure that they are protected from environmental contamination).
