    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • The written procedure for antiseptic use should be updated when a new antiseptic product is obtained.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • The antiseptic product used for skin preparation was not acceptable - no DIN or NPN was available. Please obtain a new antiseptic product.

    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Facility specific written procedures for skin cleansing, antiseptic use, and post-service care, and sink decomtamination was not avaialble.Please ensure the aforementioned procedures are in place. This can be achieved by completing pages 2-4 and page 6 of the AHS written procedures template that was left onsite.
    • 20. Is reusable equipment disinfected properly?
      • Preempt HLD5 high level disinfectant was used, however manufacturer's instructions for use (MIFU) for daily testing of efficacy was not being done.Please ensure appropriate test strips (AHP2) is acquired, used daily, and results are logged.
