      • The owner of the personal service business, Khaviar Hair, did not submit a notification about the business to the Alberta Health Services. Submit the notification to AHS.

      • The owner of the personal service business, The Extensionist, did not submit a notification about the business to the Alberta Health Services. Submitted during the inspection.
    • 17. Is reusable equipment cleaned properly (Critical)?
      • Two hairbrushes observed in the clean storage (Khaviar Hair area) were not visibly clean (had hair). Please ensure that all re-usable tools are cleaned and disinfected after each client.
    • 20. Is reusable equipment disinfected properly?
      • No approved disinfectant was available for surface disinfection: the product used did not have a DIN and disinfectant claim. Obtain an approved disinfectant.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Re-usable glasses were used for clients' service. Remove the re-usable glasses or provide proper dishwashing equipment.
