      • April 21 Re‑Inspection: Cleaning behind the shelving unit containing the UV sterilizing cabinets is required, as additional droppings were observed. Please ensure the cabinet is moved away from the wall to allow for proper cleaning, as the visible debris suggests a thorough cleaning was not completed.The facility operator noted that the mall has weekly pest control services every Thursday. Please ensure all droppings are cleaned as previously instructed and continue to monitor this area, as well as the rest of the facility.March 17 Inspection:Mouse droppings were found on the floor behind the shelving unit containing the UV sterilizing cabinets. Please safely remove the droppings and clean and disinfect the affected areas. Continue to monitor for any further droppings or signs of mouse activity.

    • 06. Do operators and personal services workers comply with the Personal Services Regulation and Standards and do they have the skills and knowledge to perform their duties in a safe and sanitary manner?
      • Knowledge of semi‑critical manicure and pedicure tools, appropriate disinfectants, and proper cleaning and disinfection processes requires improvement among staff.Cleaning and disinfection process, and the correct use of disinfectants were discussed during the inspection. Information documents will be emailed, and a follow‑up will be required to confirm understanding.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Visibly used and worn nail buffers and nail files were observed in the manicure station drawers. All used nail buffers and nail files were discarded during the inspection.Please ensure that all single‑use items are discarded after each client use. This includes drill bands, nail buffers, nail files, toe separators, and disposable pedicure flip‑flops, as these items cannot be cleaned or disinfected.
    • 16. Is reusable equipment handled and stored properly (Non-Critical)?
      • The linen used to cover the client bed in the waxing room was visibly dirty and in poor condition. In addition, the client bed was discoloured in the crevices along the sides.Please use single‑use paper to cover the bed and discard it after each client. The client bed must be cleaned and disinfected after every use. If linen bed covers are used, they must be removed after each client and laundered before reuse.

      • The following deficiencies were noted:1. High‑level disinfectant logs were incomplete and not being filled out daily.Please ensure that if the high‑level disinfectant is being re‑used, the manufacturer’s instructions are followed, including daily testing, recording pass/fail results, and documenting the date the solution is discarded.2. It was unclear whether semi‑critical manicure and pedicure tools are being properly cleaned with soap and water, dried, and fully submerged in high‑level disinfectant (HLD5) for 5 minutes. Staff reported using Preempt RTU spray or a sterilizing cabinet instead.Semi‑critical manicure and pedicure tools must be cleaned and then disinfected using high‑level disinfectant (HLD5). Pedicure basins and other surfaces may be disinfected with products such as Mr. Clean, Lysol sprays, or Preempt RTU after an initial cleaning step.

      • Some cleaning is required throughout the facility, particularly on floors, baseboards, and hard‑to‑reach areas such as behind or underneath workstations, storage areas, and cabinets.1. Equipment such as nail clippers/foot files were soaked in preempt CS20 for only 5 minutes.-Please follow manufacturer's instruction for usage, equipment/tools should be fully immersed in preempt CS20 for 20 minutes.3. High-level disinfectant concentration not regularly monitored-Please regular monitor the high-level disinfectant concentration and ensure its test result is documented as required.
