    • 20. Is reusable equipment disinfected properly?
      • 1. The operator is reusing the high-level disinfectant solution for 14 days. No test strips were available. Test strips are required to determine if the concentration of the high-level disinfectant solution is effective to reuse. Obtain accelerated hydrogen peroxide 7% test strips to test the Preempt CS20 solution. 2. No disinfection logs for the high-level disinfectant. Ensure disinfectant solutions are tested and values are recorded daily. Keep a record of the concentrations onsite. An AHS template was provided to the operator.3. The PREempt wipes was expired. No unexpired wipes available onsite to replace at the time of inspection.- Please discard and obtain unexpired ones.
    • 20. Is reusable equipment disinfected properly?
      • Contact time for high-level disinfectant is not in accordance with the manufacturer instructions. Instruments are being immersed in CS20 for only 3 minutes instead of the required 20 minutes.- Ensure that semi-critical tools are fully immersed in undiluted CS20 for 20 minutes. This prevents inadequate disinfection, thereby increasing the risk of infection transmission.
