    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • Hair services were the only service reported to AHS. Tools and equipment were observed onsite for manicures, pedicures and facials.
    • 20. Is reusable equipment disinfected properly?
      • 1. Approved disinfectants were not onsite for disinfecting semi-critical tools and semi critical tools were found in the uv storage unit.2. An approved disinfectant was not onsite for disinfecting the pedicure basin.-Do not offer any esthetic services (manicures, pedicures, facials) until an inspection is done to review procedures and proper disinfectants are obtained.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • There was a buildup of hair on the floor and in drawers in the salon.-Clean the floor and all drawers, including equipment in the drawers.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • The hand wash/equipment cleaning sink in the salon was being used as a food prep sink and for washing dishes as there was food debris in the sink, a lot of food in the salon, and re-usable dishes stored around the sink and in cupboards.-Remove all personal food and dishes from the salon and do not prepare food or wash dishes in the hand wash sink. The salon only has one sink, which can only be used for hand washing and equipment cleaning.
