- The sink in the therapeutic massage room leaks when turned on. Owner is aware of the issue and is looking into getting the plumbing fixed prior to the next massage appointment. As the sole masseuse is away for the holidays, the plumbing will be repaired sometime early next year. Ensure that all equipment in the facility is maintained in good repair.

- 05. Are written procedures reviewed and revised when required, and do they meet the requirements set out in the standards?
- Written cleaning and disinfection procedures of semi-critical reusable equipment were not available for review onsite. These procedures must be available onsite for staff to reference during the cleaning and disinfection process of semi-critical reusable equipment. This is to ensure that all staff have access to current, relevant instructions on how to clean and disinfect equipment used at the facility.

- Used electronic nail file drill bits (single-use) were found stored in the same container as unused drill bits in the reprocessing room. Used single-use drill bits are required to be disposed of after use on a single client. Additionally, unused drill bits should be kept in a separate location of used drill bits to minimize chance of cross-contamination between clients. Ensure that used drill bits are discarded and unused bits are kept clean and sanitary until they are used on a client.

- Records of high-level disinfection logs were unavailable/incomplete at the time of inspection. No high-level disinfection logs were available for the month of October 2025. Ensure that the high-level disinfection logs are maintained on a daily basis and that they are available onsite for review.

- The storage space available in the reprocessing room did not appear to be sufficient to store all required equipment and products in an organized manner. Multiple cupboards in the storage compartment were disorganized and had a mix of single-use, semi-critical, and reusable equipment.

- 1. The container holding the high-level disinfectant (CS20) was observed to be soiled. The solution is required to be replaced once a day or whenever the solution becomes too contaminated to properly disinfect semi-critical instruments. Ensure that the solution is changed at least once a day and as required based on usage.2. Semi-critical instruments such as a manicure drill bit, was observed stored in an unclean condition. Ensure that these instruments are stored in a separate designated location, until they are cleaned and disinfected.

- Operators on site did not have knowledge on the location of the high level disinfectant and written procedures within the facility.Ensure that at least one person in the facility is trained and knowledgeable on the whereabouts of the items mentioned above at all times.It was discussed with the facility owner via phone, that she will train the operators about the location of the high level disinfectant and the binder of written procedures.

- A used nail sanding band was left on a nail drill bit. Discard all single use items immediately after it was used on a client.

- Semi-critical equipment such as tweezers used for eyelash extensions in the service room and manicure drill bits were observed dirty. Clean and disinfect semi-critical tool and ensure all tools are visually inspected after cleaning.

- High level disinfectant was not observed in the facility. Operators did not know where the bottle of CS20 was during the inspection. Ensure that a high level disinfectant is available and used to disinfect semi-critical tools and equipment. Note: Through a phone call, the owner stated there is CS20 available in the facility, but only she knows where.

- Bottles and containers used for client services were unlabeled. Such as a squeeze bottle in the massage room, acrylic nail solutions in the first manicure station, spray bottles of CS20.Clearly label all bottles and containers that are used for client service.
