      • The station on the left side of the facility as you walk in had combs, scissors, and other instruments not fully immersed in barbicide. The operator replaced the solution and filled the container so all instruments were fully immersed in the disinfectant. Ensure that all instruments are fully immersed when not in use.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • The station in the rear right corner when you walk in had its floor mat in poor condition. There were holes in the lining and the exposed surfaces were not able to be cleaned effectively and trap hair. Ensure that the mat is replaced or repaired such that it is smooth, non-absorbent, and easily cleanable.

    • 17. Is reusable equipment cleaned properly (Critical)?
      • Clean hair clippers throughout the facility had significant hair debris. Clean and disinfect all reusable tools and equipment such as hair clippers after each customer use.
    • 20. Is reusable equipment disinfected properly?
      • Facility operator demonstrated reprocessing procedures of reusable equipment and tools. Low level disinfectant (LLD), Barbacide, was not used as manufacturing instructions directed. LLD directions for use were reviewed with the facility operator. Ensure that all disinfectants manufacturing instructions for use are followed for effective disinfection.
