- The owner of the personal service business did not submit a notification about the business to the Alberta Health Services. Submit the notification.

- The operator demonstrated the lack of knowledge regarding the personal services requirements, including re-usable tools reprocessing, disinfectant use and classification, cleaning and disinfection of pedicure baths, etc.

- No skin cleansing was conducted prior to waxing and eyelash extensions services, and no cleansing product was available.

- No cleaning tools were available for cleaning of re-usable tools prior to disinfection. Obtain proper cleaning brushes to ensure that the tools are clean prior to disinfection. A round brush is recommended for cleaning of nail clippers.

- No soaking tray was used for disinfection of the tools. The semi-critical tools must be disinfected by soaking in a high-level disinfectant.

- 1. Double-dipping during waxing procedures was observed: multiple used waxing strips and only 3 wooden spatulas were observed in the garbage bin.2. The spatula used to obtain the scrub from the bulk supply during the pedicure services was being re-used.

- A ceiling tile was missing in the reprocessing area room. Replace the missing tile.

- The reprocessing sink located in the back staff room was also used for dish washing (dishes observed). The reprocessing sink must be designated for tools reprocessing only and cannot be used for dish washing.
