    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures were completed during the inspection.Written procedures for cleansing skin has been completed for the following services:-manicure-pedicure-waxing-eyelash extension-eyelash tint-eyelash liftWritten procedures for accidental skin cuts or punctures was provided to the operator at the time of inspection.Written procedures for handwashing and equipment reprocessing sink decontamination was completed at the time of inspection.
    • 20. Is reusable equipment disinfected properly?
      • Semi-critical equipment (i.e. large metal foot files) were not being properly disinfected. They were not being fully immersed in the disinfectant. The tray was too small. Obtain a suitably sized, inexpensive plastic/glass tray with a lid to soak the semi-critical tools in the high-level disinfectant (HLD). You must be able to visually confirm tools are being fully immersed.
    • 20. Is reusable equipment disinfected properly?
      • High-level disinfectant logs (HLDL) templates were provided to the facility and properly filled out at the time of inspection. Discussed how to properly maintain HLDL at the time of inspection.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Some cosmetic products in the waxing room were requested to be removed from the facility because it was not properly labelled and/or missing information. All cosmetic products used must have the following information in the English language: product identity, list of ingredients, instructions for use.

      • 1. The Stacy Lash Extra Strong Evolution professional eyelash extension glue was missing label information. A list of ingredients was not observed on the glue bottle and may be on the original packaging. Obtain further information as needed. Cosmetic products must have the following labels: product identity, list of ingredients & instructions for use.2. Dirty spoons/sticks were being left in cosmetic products at the pedicure station which may contaminate the bulk supply. Discard spoons/sticks after each use.3. Cosmetic products for pedicure services were not labelled with its content (i.e. pedicure salt, pedicure gel etc.). Label all containers with its contents and ensure the original/bulk supply containers are kept on-site.4. Spray bottles containing isopropyl alcohol at the manicure stations were not labelled with its contents. Label all containers with its contents and ensure the original/bulk supply containers are kept on-site.5. The squeeze bottle in the waxing room used for eyelash extension services is not labelled and allegedly containers tap water. Label the squeeze bottle with its contents. Change the tap water on a regular basis.
