      • Some single use equipment appears to be re-used. Single use equipment is to be used once and discarded. This includes Dremel bit sandpaper, and nail files.

      • 1. Equipment is being cleaned and disinfected and then restored in contaminated containers. Containers holding clean equipment must be cleaned and disinfected between use.2. Pedicure basin recirculatory jets are not being properly dissembled and cleaned between use. They are to be fully cleaned and disinfected with a low-level disinfectant.

      • Contaminated equipment is being stored with single use, and clean equipment. Ensure there is a separation of used from unused equipment. Prevent equipment from contamination at all times.

      • Physical residual was observed on items that were considered re-processed. If staff cannot properly clean items prior to disinfection, consider an ultrasonic cleaner.Items are to be fully cleaned prior to disinfection.

      • Semi-critical equipment was being re-processed at the washroom sink.Operator instructed to not use washroom sink for reprocessing equipment.

      • Written procedures were not observed. A copy of the template was provided to the operator.

      • Records of disinfectant testing not observed. This was corrected onsite.High level disinfectant failed the test with the test strips. This was corrected onsite by the operator.
