      • Dried debris noted on the tools stored in the UV cabinet. Tools were not sufficient cleaned and inspected prior to the next step. - Ensure all tools are visually inspected and properly cleaned before disinfection.
    • 20. Is reusable equipment disinfected properly?
      • 1. All Preempt CS20 and RTU solutions were expired in 2024 and 2025.Unknown solution was found in the disinfectant tray. Failed the H2O2 test.- Obtain valid disinfectant solution. Disinfect all the semicritical tools onsite before reopening to the public. 2. Manufacturing instructions for the Preempt CS20 were not followed: tools were not visually inspected after cleaning, tools were not dried before soaking into solution, tools were soaked for about 15 mins, disinfectant were reused more than 1 day without verified concentration. - Follow manufacturing instructions, and post a step by step process at the reprocessing area.a. Clean the tools with soap and brush.b. Visually inspect the tools for debris.c. Dry the tools in clean cloth before soaking into CS20.d. Soak the tools for 20 minutes.e. Remove the tools, rinse the tools, and dry the tools with a clean cloth.f. Store the tools in dry and clean containers.

      • The sharp container was over filled with used callus blade stored on the lid.- Educated on the maximum fill line, and immediately dispose of the container safety (ie, local pharmacy or any other sharp container dispose site.).
