- 1-The high-level disinfectant (HLD) CS20 was not tested prior to re-use. High level disinfectant specific Test strips were not available to do daily testing.2-HLD testing records were not available for review.Obtain HLD test strips and perform daily HLD testing prior to re-use and record the test results in Daily HLD testing log. Template of Daily HLD testing log was provided onsite.

- Facility specific written procedures were not completed for1-client care in case of accidental cut and injury, 2-decontamination of dual-purpose sink.3- preparation of skin - Skin cleansing AHS template hard copies were provided to the operator to complete and email the copies to PHI.

- A low-level disinfectant at minimum is required to disinfect the reprocessing sink prior to reprocessing of semi-critical tools. Obtain a low-level disinfectant at minimum to disinfect the sink and environmental surfaces.
- 31. Are sinks accessible and used appropriately for the personal services provided?
- Hand washing is reportedly being done in the reprocessing sink located in the room. A decontamination of dual purpose sink procedure is required. Client must wash their hands in the washroom hand sink.
