Inspection summary

25th Avenue has been inspected 9 times by Interior Health Authority since August 6, 2015. The most recent inspection, on July 29, 2026, found no infractions.

1 of 9 inspections on record found no infractions at all. In the past 12 months there was 1 inspection with 0 infractions recorded. All recorded findings relate to upkeep, equipment or paperwork rather than direct health risks.

Recent inspections have recorded fewer infractions than earlier ones, so the record is improving.

What these findings mean

Health authorities word their findings differently, so we group them by what they mean for the people in care. Higher-risk findings are the ones that could directly make someone sick, and they matter more here because young children, older adults and people who are unwell are more vulnerable to infection.

  • Permits, records and postingLower risk

Operating permits, required signage, written procedures and logs. These findings are administrative: the documentation was missing or out of date at the time of inspection.

Recorded 7 times across 5 separate inspections, most recently on June 27, 2025. For example: 1 - Are person in care records current, complete and kept confidential?

  • Equipment and utensilsLower risk

Equipment has to be made of cleanable materials, kept in good repair and stored properly. These findings are mostly about upkeep, like a cracked surface or worn part. They are generally lower risk but show how well the place is maintained.

Recorded 2 times across 2 separate inspections, most recently on August 11, 2016. For example: 1 Are the physical facility, equipment and furnishings maintained, sanitary, accessible and appropriate for persons in care?

  • Other findingsLower risk

Findings that do not fit the categories above, such as follow-ups that confirm an earlier violation was corrected, or requirements specific to this type of facility.

Recorded 7 times across 5 separate inspections, most recently on April 26, 2024. For example: 1 - Are medications stored, handled, and administered appropriately?

  1. Routine Inspection

    0 infractions

  2. Routine Inspection

    2 infractions

    • R4.1 - Are person in care records current, complete and kept confidential?
      • Observation(s): During the inspection of the physical facility, it was noted that person in care records were accessible in an unlocked cabinet in a common area accessible by persons in care, and visitors. Historical bank statements belonging to persons in care were noted to be accessible in the unlocked cabinet. Failure to secure personal information poses a risk to the confidentiality and privacy of persons in care. This contravention was addressed during the inspection, with the immediate removal of personal records and their placement in a locked room accessible only to employees. The licensee will submit a plan by July 11, 2025, outlining the system that will be implemented to ensure that all person in care records are securely stored in accordance with regulatory requirements. The plan should detail how records will be protected from unauthorized access, including the procedures for secure storage, staff access protocols, and any monitoring or review processes that will support ongoing compliance.
      • R4.1V - Keep the records and personal information of persons in care confidential to the greatest extent possible while maintaining the health, safety and dignity of persons in care; 93
    • R10.3 - Do care plans include the required elements and are they developed with the participation of the person in care, parent or representative?
      • Observation(s): During a review of medication care plans, medication administration records (MARs) and overall care plans, it was noted that one person in care's medication care plan did not reflect the most current information. Specifically, the MAR included a sticky note indicating that pills were to be crushed; however, this instruction was not documented in the medication care plan, nor was the note dated or signed. The presence of an undated and unsigned sticky note on the medication administration record (MAR) indicating instructions, without corresponding documentation in the medication care plan, poses a risk to safe medication practices. This contravention was addressed and corrected during the inspection. The medication care plan was updated to include the current instructions regarding medication administration, ensuring alignment with the information previously noted on the MAR. The undocumented sticky note was removed, and the proper documentation process was followed. The licensee will submit a plan by July 11, 2025, outlining the system that will be implemented to ensure ongoing compliance with documentation requirements related to medication care plans and administration records. The plan should describe the processes in place to ensure that any changes to medication instructions are accurately documented, properly authorized, and consistently communicated to relevant staff, and any monitoring or review processes that will support ongoing compliance.
