UNIT OFF AND NOT MAINTAINING PROPER TEMP PER MANAGER. OBSERVED SLICED TOMATOES, SOUR CREAM AND TAHINI SAUCE IN UNIT. INSTRUCTED TO REMOVE ITEMS FROM UNIT AND TO HAVE UNIT REPAIRED TO HOLD FOODS AT 40F AND BELOW.

OBSERVED HUNDREDS OF MOUSE DROPPINGS IN BOXES WITH POTATO CHIPS, ON SHELVES UNDER FRONT COUNTER, ON SHELVES WITH JAR AND CANNED FOODS, UNDER FLOOR SHELVES, INSIDE BATHROOM CABINET, IN REAR STORAGE, ETC. ALSO OBSERVED A DEAD MOUSE BEHIND REFRIGERATOR. MUST CLEAN ALL AFFECTED AREAS. PEST CONTROL REPORTS INDICATE ACTIVITY FOR THE LAST THREE MONTHS.

- The walls and ceilings shall be in good repair and easily cleaned. OBSERVED LARGE HOLES IN WALLS THROUGHOUT ESTABLISHMENT UNDER SHELVES AND ABOVE SHELVES, BEHIND EQUIPMENT AND UNDER SINK IN BATHROOM. MUST SEAL ALL OPENINGS.
