      • Observation: The Permit To Operate is not in the premises. The decal is.
      • Corrective Action: Obtain the Permit, attache the decal to it and post it in a conspicuous location in the facility.

      • Observation: The outside of the tub containing filter powder has accumulated dirt on it.
      • Corrective Action: Cleaned the outside of the tub thoroughly, daily.
