- In food establishments, there shall be adequate sewage and waste water disposal facilities that comply with all the requirements of the plumbing section of the Municipal Code of Chicago. MUST PROPERLY INSTALL A MOP SINK WITH HOT/COLD WATER AND BACK FLOW DEVICE.
- 12. HAND WASHING FACILITIES: WITH SOAP AND SANITARY HAND DRYING DEVICES, CONVENIENT AND ACCESSIBLE TO FOOD PREP AREA
- Adequate and convenient hand washing facilities shall be provided for all employees. MUST PROVIDE SOAP AND HAND DRYING DEVICES IN WASHROOM.

OBSERVED MICE ACTIVITY THRU-OUT PREMISES; MICE DROPPINGS NOTED ON FLOORS, ALONG WALLS ON SHELVES, UNDER SHELVES, UNDER COOLERS, THRU-OUT PREMISES OVER 200 MICE DROPPINGS NOTED. MUST CLEAN/SANITIZE THRU-OUT PREMISES. CALL PEST CONTROL TO SERVICE BUSINESS AND SEAL ALL OPENINGS ON WALLS, CEILING AND FLOORS TO PREVENT PEST ENTRY.

- The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. BROKEN FLOOR TILES, MUST BE REPLACED AND CLEAN FLOORS THRU-OUT PREMISES INCLUDING BASEMENT. ALSO REPAIR WASHROOM FLOOR AND SEAL OPENINGS.
- 35. WALLS, CEILINGS, ATTACHED EQUIPMENT CONSTRUCTED PER CODE: GOOD REPAIR, SURFACES CLEAN AND DUST-LESS CLEANING METHODS
- The walls and ceilings shall be in good repair and easily cleaned. MUST REPAIR AND REPAINT WALLS AND CEILING IN WASHROOM, SEAL OPENINGS THRU-OUT PREMISES TO PREVENT PEST ENTRY. ALSO REPLACE DAMAGED CEILING PANELS WHERE NEEDED.
- 38. VENTILATION: ROOMS AND EQUIPMENT VENTED AS REQUIRED: PLUMBING: INSTALLED AND MAINTAINED
- Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REPAIR OR REPLACE TOILET BOWL.
