      • First aid kits throughout the facility needed to be restocked. First aid kits were also missing antiseptic wipes. Please go through first aid kits and ensure they are restocked and up to date on required material.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. Finishes throughout the we damaged and in need of repair. Please repair/refinish the following areas to ensure surfaces are smooth, non-absorbent, and easy to clean.- Finish on the wall in the Lions rooms- The pillar in the chipmunk room- The wall by the cots and the wall by the windows in the Lady Bug Room- The finishes on the vendor stalls in the Village Square 2. The top of the divider in the chipmunk room was damaged and held with tape. Please repair/replace the divider to ensure surfaces are smooth, non-absorbent, and easy to clean

      • Bleach water solutions were not reflective of the concentration listed on the bottle in the described rooms. - Whales Room: 1000ppm in 100ppm bottle- Lions Room: 1000ppm in 100ppm bottle- Sea Horse Room: 50 ppm in 100 ppm bottlePlease ensure the bleach water concentration accurately reflects the labeling on the bottle so effective disinfection is achieved.

      • Staples were being used in the following rooms, which can create a sharps and choking hazard.- Chipmunk Room- Bunnie Room- Squirrel RoomPlease ensure staples are either covered with tape or removed from the listed rooms.

      • First aid kits throughout the facility needed to be restocked. First aid kits were also missing antiseptic wipes. Please go through first aid kits and ensure they are restocked and up to date on required material.

      • Bedsheets stored on cots were overhanging and making contact with other children's bedsheet. The following rooms had overhanging bedsheets- Busy Bees- Lady Bug RoomOperator moved the bedsheets to ensure they were stored separately.

      • Children's personal items were not properly stored in the cubbies, resulting in items not being separated. Improper storage was observed in the following rooms- Butterfly room- Ladybugs room- Squirrel room- Bunny Room-Raccoon Room- Whale Room- Seahorse RoomPlease store children's personal items inside of the cubbies to ensure they remain separated

      • 1. Diaper change mat in Busy Bees is in disrepair. Please replace. 2. Diaper change areas and hard to reach areas (under mats and corners of classrooms) have a buildup of debris. Please clean areas on a regular basis (weekly).
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Some toys are uncleanable (yellow doll house in Bears, wood/cloth beds in Village, cardboard boxes in Bunny room storing toys, tape on blocks in Ladybugs) or are visibly dirty (Butterfly, Busy Bees).
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • 1. Sunbeams room had a lot of clutter and personal items in the washroom. Please clear out the room. 2. Storage for bedding is touching the underside of the cots. Ensure that bedding is flat or store in their cubbies.3. Ladybugs room had a bin full of jackets and hats that were stored until the staff could return them to their cubbies. Do not store personal items together.

      • Bedding was stored on top of cots, or were touching adjacent cots. Please ensure that bedding and sheets are stored in a manner that prevents them from touching other childrens' belongings, including cots.
