    • 18. NO EVIDENCE OF RODENT OR INSECT OUTER OPENINGS PROTECTED/RODENT PROOFED, A WRITTEN LOG SHALL BE MAINTAINED AVAILABLE TO THE INSPECTORS
      • Improper pest control methods. Large amounts of litter, garbage, toilet, food containers on outside ground, stairwell and basement of premises. Holes/burrows in ground behind establishment. No pest control log book available at time of inspection. Small gaps to outside at front door. Hearing notice issued. 7-38-020. Serious violation. Must clean/remove debris from outside rear, stairwell and basement of premises, seal holes/burrows and repair front door. Have premises serviced by a licensed pest control company and provide log book.

      • Walls through-out establishment not clean. Must clean and maintain walls. Water stained/damaged ceiling tiles. Must repair any leaks and replace ceiling tiles.
