- Holes on the ceiling were observed in disrepair exposing drywall and evidence of water damage, near the ceiling heating vent and near the range hood. Repair or refurnish the ceiling to ensure a smooth, durable, and easy to clean surface. **OUTSTANDING VIOLATION**Update March 26, 2026: The ceiling holes near the range hood were observed filled with an unsmooth and uneven finish. The ceiling hole near the heating vent was still observed.

- The ventilation rangehood was not functional at the time of inspection. Facility operator indicated that if the rangehood is turned on it immediately cuts out and a new motor has been ordered.Ensure that the ventilation rangehood is functional. Provide a copy of the receipt or invoice of the new motor and the estimated timeline of installation.

- Food items including canned soups and a box of hotdog buns were stored on directly on the floor. The food items were moved onto a shelving. Ensure that all foods are stored above the floor to prevent contamination.

- Ceiling air vents along the walls of the kitchen were observed with accumulated dust. Clean the air vents to prevent food contamination.
