  - Cooked chicken found on the hot holding table was not reheated to required temperature, measured at 40C.- Ensure chicken is reheated to 74C before hot holding. Education provided, chicken reheated again.

  - Pest control monitoring was conducted in house, but no record was retained.- Pest control checklist sent by email, ensure the monitoring record is maintained.
- 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
  - A tile is missing on the floor near the dishwashing area. The baseboard near the microwave area came off. - Ensure the areas are repaired for smooth, easily cleanable surfaces.

  - 2 one-compartment sinks were used for dishwashing in the kitchen.- Ensure a two-compartment sink is installed to meet requirements.

  - Beef and chicken hot holding measured to be 36-40CThe meat (approximately 2lbs) was discarded during inspection.Keep the hot food above 60C
- 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
  - A few of the wooden shelves (especially bottom shelves) in the kitchen are worn out.Ensure to repair or replace the shelves so that they are smooth, impervious to moisture and easily cleanable.
