- Light fixture without cover was observed in baby's room.-Please provide appropriate cover for the identified light fixture or ensure the light bulbs are shatterproof to prevent potential injury in the of breakage.

- Extraneous materials were being kept on diapering surfaces.-Please remove all materials kept on diapering surfaces to ensure proper cleaning and sanitizing of the surfaces and also prevent bulk contamination of diapering materials.

- First aid kits with expired antiseptic wipes and missing emergency blankets were noted onsite. -Please provide emergency blankets and new antiseptic wipes in all classrooms first aid kits.

- Sensory bottles with multiple layers of adhesive tapes making them harder to clean/disinfect properly. -Please ensure that the bottles are visibly clean. Advised to use non-toxic glue instead.

- The chlorine sanitizer in some rooms measured over 200 ppm. Staff were not able to indicate whether it was rinsed off of surfaces that children would come into contact with.- Confirm the concentration(s) of surface sanitizer expected to be used in the facility. If using chlorine sanitizer >200 ppm, surfaces and objects that children touch must be rinsed after. Ensure staff are trained to use sanitizer appropriately
