      • Observation: Slime and black spots noted inside the ice maker ice tray. Operator stated that ice maker will be serviced today.
      • Corrective Action(s): Clean and sanitize ice trays and ice bin. Please follow steps below to ensure ice is protected from potential contaminant.
      • - Empty ice bin.
      • - Wash and sanitize ice bin using 100-200 ppm bleach water solution. Making solution discussed with operator.
      • - Clean and sanitize ice making aparatus, grid and water intake containers above the ice bin.
      • - Ensure no black/green spots left on surfaces.

      • Reminder: Ensure to refill the sanitizer bottle after testing the solution from the dispenser. QUAT 200-400 ppm must be detected in the solution.

      • Observation: Dishwasher detergent bucket was empty at time of inspection.
      • Ensure that counters and cuttingboards are being sanitized throughout the day and between tasks. Sanitizer bottle was measured and found to have 100 PPM Quat residual. Ensure that dispenser flushed prior to filling bottles.
      • Corrective Action(s): Replace bucket with new one and prime machine. Do not wash dishes until detergent is running through dishwasher.
      • Flush sanitizer dispenser prior to filling bottles. This was done and concentration of Quat measured 200 PPM.
      • Violation Score: 15

      • Observation (CORRECTED DURING INSPECTION): Ensure that raw meats are stored below ready to eat foods in all coolers.
      • Corrective Action(s): Raw meats are now being stored on the lower shelves. Continue to maintain.
