- The bleach sanitizer solution in a spray bottle is noted to be contaminated with some other chemical solution. Ensure that bleach sanitizer solution is prepared to be at 200 pm with plain water and not mixed with any other chemical. Bleach sanitizer test strips must be made available to test concentration of the solution.

- A QUAT sanitizer solution that is very strong and concentrated is being used. A solution of bleach at 200 ppm must be prepared by the operator and brought with them to any site where they are conducting food preparation. The QUAT sanitizer solution was adjusted and measured to be at 200ppm during the inspection.

- Foods are transported in an insulated fabric bag that is not smooth and easily cleanable.Ensure that the foods transported to the food facility for preparation are in a cooler that is smooth and easily cleanable with ice packs.

- The owner is operating in contravention of the permit conditions by indicating that some food preparation and baking is performed from a personal kitchen. Ensure that the all food handling, other than service is occurring in an approved food establishment as is indicated on the permit.
