      • There was untreated wood by the dual-purpose sink.Seal untreated wood. Surfaces must be smooth, easily cleanable and waterproof.

      • There were no test strips for monitoring the high-level disinfectant during the reuse period.Provide test strips.

      • The client chair at the shampoo sink was dirty.Clean and disinfect the client chair.

      • There was no water supply to the dual-purpose sink. Water lines had froze.Provide a continuous supply of hot and cold running water at all times. Take adequate steps to ensure that water supplies and wastewater lines do not freeze up during cold weather.

      • Large pieces of fabric were being used for separating the personal services side from personal space in the garage. Walls shall be easy to clean and maintain.

Shelf liners that are easily cleanable can be used.

      • Written procedures were not available for the following- cleansing of skin and mucous membrane- client care in the event of accidental skin cut or puncture- decontamination of any sink that will be used for handwashing and equipment reprocessingComplete written procedures. AHS templates were provided.

      • Based on the violations noted during this inspection, it is evident that the operator did not demonstrate an understanding of Personal Services Standards/ Regulation.

      • There was no paper towel holder/ dispenser at the handwashing sink.Provide a paper towel holder/ dispenser.

      • A used nail buffer and lash brushes contaminated with lint were found in storage. The operator indicated that these items were for personal use.Used disposable items were discarded. Do not re-use single-use items and keep personal items separately.

      • 1) Combs and hair styling tools were not completely immersed in barbicide.Reusable instruments must be completely immersed in the disinfectant.2) Barbicide concentrate was not properly diluted with water as per manufacturer's instructions.Obtain measuring cups and spoons and follow manufacturer's instructions for preparing the barbicide. 3) A high-level disinfectant was not available for facial extraction tools.4) A disinfectant tray was not available.Provide a high-level disinfectant and a disinfectant tray. Follow manufacturer's instructions including the reuse period and contact time. Monitor the disinfectant with test strips daily and maintain a written log.

      • The concrete floor was unfinished. There were holes and exposed insulation in the walls. Walls and floors shall be easy to clean and maintain.

      • The reprocessing area did not have adequate space to separate clean and dirty equipment. Provide adequate space for- cleaning and disinfecting equipment- separating clean and dirty equipment- facilitating one-way flow of dirty to clean equipment

      • Operator did not demonstrate proper understanding of Personal Services Standards. This includes but not limited to failure to maintain/develop the facility in a sanitary condition, improper setup of service area, and lack of required infrastructure.

      • Buildup of dust and debris noted on workstation and clipper guards.- Please thoroughly clean the hair salon equipment and make sure it is cleaned on a frequent basis.

      • No disinfectant was available for cleaning and disinfecting environmental surfaces at the time of inspection.- Obtain low-level disinfectant for use on all environmental surfaces between clients. Disinfectant must have a Drug Identification Number (DIN) and be used according to manufacturer instructions for contact time and dilution.

      • 1. The personal service facility was not maintained in good repair. The space included exposed pipes, utility equipment, unfinished walls, and ceilings, and a concrete garage floor.2.
