      • Written procedures were not established for skin cleansing, antiseptic use, post-service care, and sink decontamination.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Sterility assurance documentation for all critical equipment (tattoo cartridges) used during each service were not retained as required.
    • 20. Is reusable equipment disinfected properly?
      • Semi-critical equipment such as lash extension tweezers were not disinfected with a high level disinfectant.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • Reusable equipment was being reprocessed in the kitchen sink. Going forward, the stainless steel laundry sink on the second floor of the house will be used to reprocess all reusable semi-critical, and non-critical equimpent. A procedure will be established to decontaminate the sink before and after the area is used for reprocessing equipment such as lash extension tweezers.
