    • 06. Are proper protocols being followed in regards to client care?
      • Chlorine test papers were not available onsite for testing the bleach water solution concentration during the toy washing . Please make sure Chlorine test papers are available .
    • 08. Is this facility properly constructed (Non-Critical)?
      • * Wallpaper on some of wall surface in the downstairs room were peeling off.Surfaces are to be maintained in good repair to ensure they remain smooth, cleanable, and free from damage. Please make sure repair or remove the peeling materials.* Floor tile in one of the washrooms beside the leader's washroom upstairs room was lifting.Please make sure it is properly repaired.* Plastic baseboard by the corner wall beside the leader washroom area in the downstairs was lifting.Please make sure it is repaired.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • *Play center in downstairs room surfaces were not in good repair. Nails and staples on the playcenter were sticking out, surface was not smooth, and cleanable.***Art supply storage room* Surfaces in the art supply room/ storage room was not in clean and good repair. Please make sure areas in the rooms is organized and cleaned. * Flooring material peeled off and damaged/ not in good repair. Please make sure repair of the areas, and cleaning is completed.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • Upstairs washrooms did not appear to have mechanical ventilation working. No noise was heard upon activating lights, and vent did not appear to pull air out from room. Ensure washrooms have working ventilation.

    • 10. Is a properly supplied first aid kit provided and readily available?
      • CPR Face shields were missing in some of the back bags.Please make sure they are available in the back bags.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Tall shelves in the eating area and in the common area were not mounted to wall safely. Please make sure they are mounted and secured.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • Upstairs washrooms did not appear to have mechanical ventilation working. No noise was heard upon activating lights, and vent did not appear to pull air out from room. Ensure washrooms have working ventilation.

    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • One of the carpets in the small room in the basement was not in good repair.*** Please repair/replace the carpet.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • Play house near the window in the small room was not secured.***Please make sure it is safely secured to prevent any injuries.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Grey play mat in the basement was not in a good repair. It was ripped/damaged and taped on some of the areas on. It was not in clean condition.***Please replace it. *Play tray under the playhouse was not in clean condition. There were lot of food debris in the play tray.****Please clean it.
