      • Observed the floor of the walk in coolers to be rough and damaged in certain areas. Instructed facility to repair and maintain smooth and easily cleanable. Observed dirt and debris on the floor in various areas: in the basement throughout, behind shelving, and along walls and under equipment. Instructed facility to clean and maintain.

      • Observed water stained ceiling tiles in various areas. Instructed facility to replace and maintain.

      • Observed construction debris/equipment and waste scattered in the rear of the basement. Instructed facility to remove any unnecessary items, organize remaining, store at least 6 inches off the floor, and maintain to help facilitate proper cleaning methods and help prevent pest harborage.
