- Videos sent to PHI showing semi-critical tools fully submerged in high-level disinfectant(HLD) (CS20 used in this case) with testing of the HLD with test strips also demonstrated in the video.Photos sent to PHI of the HLD Log documenting testing of the solution.However, the test strips (0-100ppm H2O2) are intended for drinking water and pool testing, and the range is not suitable for testing the 7% H2O2 solution. Operator notified that test strips are not suitable for testing CS20 and advised to check with PHI before purchasing new test strips to ensure they are the correct ones for the solution.

- Outstanding:Reusable, semi-critical instruments are being reprocessed in the bathroom. Reprocessing areas and sinks cannot be located in a room with a toilet.Please install a separate sink for reprocessing. This sink cannot be located in a room with a toilet. This sink must be equipped with running hot and cold water. Sink must also have sufficient work surface area on either side of the sink to facilitate a one way workflow to separate contaminated equipment from clean equipment and to facilitate air drying of reprocessed equipment.Please ensure reprocessing sink is installed and located in an area that separate from incompatible activities.*operator noted she is looking to install a dedicated reprocessing sink.
